EMPLOYEE COVID QUESTIONNAIRE

In light of the COVID-19 pandemic, Juliana’s (the “Company”) is taking several precautions to ensure the safety, health and general well-being of our employees, visitors and guests. At this time, these precautions include daily temperature checks, a daily COVID-19 screening questionnaire, and the use of personal protective equipment (“PPE”).
We understand that these precautions may feel intrusive and overwhelming, but we have determined them to be necessary to ensure we keep everyone at our property safe during these unprecedented times. With respect to the above practices, the Company will comply with all applicable federal, state and local laws including but not limited to health codes and privacy laws, and will continue to evaluate the guidance from government agencies and use this information to refine and revise our procedures as appropriate. All employees, guests and visitors will be subject to the same policies, regardless of age or other protected characteristic, except for those who need and are granted disability accommodation.[1]

By signing this acknowledgement form, you understand that the Company will be taking your temperature and asking you the questions on the attached questionnaire before every shift and you consent to the Company doing so. You also acknowledge that the Company has provided you with PPE, trained you on how to wear and use PPE, and you agree that you will follow all PPE and other health-related guidelines the Company currently has in place or may implement in the future.

Debido a la pandemia de COVID-19, Juliana’s Pizza está tomando varias precauciones para asegurar la seguridad, salud, y bienestar general de nuestros empleados, visitantes y clientes. En este momento, estas precauciones incluyen un cuestionario de revisión de COVID-19 para todos los visitantes y clientes. Nuestros empleados también están sujetos a revisiones diarias similares.
Entendemos que estas precauciones se puedan sentir entrometidas, pero hemos determinado que son necesarias para asegurar la seguridad de todos en nuestro local durante estos tiempos sin precedentes. Con respecto a las prácticas mencionadas arriba, Juliana’s Pizza cumplirá con todas las leyes federales, estatales y locales aplicables incluyendo, entre otros, los códigos de salud y las leyes de privacidad, y seguirá evaluando las recomendaciones de agencias gubernamentales y usar esta información para refinar y revisar nuestros procedimientos tal como sea apropiado. Todos los empleados, clientes y visitantes estarán sujetos a las mismas políticas, sin importar la edad u otra característica protegida.

Al firmar este acuse de recibo, usted entiende que Juliana’s Pizza le hará las preguntas en el cuestionario adjuntado antes de que se le dé acceso a la propiedad y que consiente con que el restaurante haga esto. También reconoce que la Empresa le ha brindado equipo protector personal (PPE), le ha entrenado en cómo se pone y se usa el PPE, y está de acuerdo que seguirá todas las recomendaciones de PPE y otras recomendaciones relacionadas con la salud que la Empresa tiene implementado actualmente o que podría implementar en el futuro.
